WAIVER AND RELEASE OF LIABILITY
Willow Creek Elementary PTA – School Carnival
I, the undersigned parent or legal guardian, give permission for my child to participate in the Willow Creek Elementary PTA School Carnival (the “Event”).
I understand that participation in carnival activities, including games, inflatables, food service, and other recreational activities, involves inherent risks of injury. I voluntarily assume all risks associated with my child’s participation.
To the fullest extent permitted by Texas law, I release and hold harmless Willow Creek Elementary PTA, Willow Creek Elementary School, the school district, and their officers, board members, volunteers, employees, agents, sponsors, and vendors from any and all claims, liabilities, damages, or expenses arising from my child’s participation in the Event, including those resulting from negligence.
I authorize emergency medical treatment for my child if necessary and understand that I am responsible for any related medical expenses.
I have read and understand this waiver and sign it voluntarily.
